A referral resource for the colleagues who trust us with their patients' care. We are accepting new patients, and we aim to make referring as simple and collaborative as possible for your practice.
Referring a patient to Bloom
We provide compassionate, whole-person outpatient psychiatric care for children, adolescents, adults and older adults, with bilingual providers who speak Spanish and Tagalog and telehealth available.
Before referral
- Confirm the patient's insurance plan
- Confirm the service being requested
- Provide accurate patient contact information
- Send relevant clinical information
What patients can expect

A referral does not guarantee an appointment, diagnosis, medication, or insurance coverage. Clinical decisions are based on the patient's individual assessment and needs. Who May Benefit From Referral
When to consider Bloom
Consider Bloom Mental Health when a patient may benefit from outpatient psychiatric assessment, diagnosis, medication management, or ongoing psychiatric follow-up.
Mood Concerns
Persistent sadness, loss of interest, mood changes, irritability, or functional decline.
Anxiety Concerns
Excessive worry, panic symptoms, avoidance, sleep disruption, or difficulty functioning.
Attention Concerns
Possible ADHD symptoms affecting school, work, relationships, or daily organization.
Mood Instability
Episodes of elevated or depressed mood, significant cycling, or concerns warranting evaluation.
Trauma-Related Symptoms
Intrusive memories, hyperarousal, avoidance, nightmares, or other trauma-associated symptoms.
Medication Needs
A patient on psychiatric medication who needs evaluation, monitoring, or adjustment.
Diagnostic Clarification
Symptoms that are complex, persistent, or unclear and would benefit from assessment.
Post-Discharge Continuity
A patient who needs outpatient psychiatric follow-up after a higher level of care.
Depression Symptoms
Persistent low mood, anhedonia, fatigue, sleep or appetite changes, or feelings of worthlessness.
Autism-Related Concerns
Social communication differences, repetitive behaviors, sensory sensitivities, or concerns warranting evaluation.
Urgent / Emergency Needs
Bloom Mental Health is an outpatient practice. Patients experiencing an immediate safety concern, medical emergency, or acute psychiatric crisis requiring a higher level of care should be directed to the appropriate emergency or crisis service rather than a routine outpatient referral.
